Select the state for which you are applying for licensure.

Please note that you will need to complete the state licensure application required by your selected state. This purchase is only for requesting the documents needed from ABSA to be completed.

If you are applying for licensure in Illinois, please ensure that the top portion of your AFF-SA form is completed prior to submission. Documents for Illinois will be sent to you by mail in a sealed envelope, which you will need to attach to your application.

For Texas, your documents will be sent directly to the Texas Medical Board.

For Washington, DC, your documents will be mailed to you in a sealed envelope, which you will need to include with your application.

For Surgical Assistant Licensure in Tennessee, Colorado, and Nebraska, you should consult your case worker to determine what documentation they require ABSA to provide.
